Waiver / liability release

I am the responsible parent or legal guardian of the participant who is under 18 years of age. By signing this document, I consent to my child or myself participating in all activities during the ATC/540 Taekwondo lesson/Program

Emergencies: In case of medical emergency, I hereby give permission for ATC/540 Taekwondo to call a medical professional and to deliver medical care should the need arise. I also understand that I am responsible for any expenses incurred for treatment and care.
If a Program is chosen, the signer agrees to pay the dues or other payments required to pay under this agreement. If the academy relocates within 5 miles, the contract must still be fulfilled. No Refunds, No Returns, No Exceptions. Classes missed can be made up only within the length of the program, no extensions will be given.

Release of Liability: I understand that my child's safety is my responsibility before, during, and after the class/event. On behalf of this child, I hereby release ATC/540 Taekwondo/Outlets at Lake Elsinore and its subsidiaries and affiliates and their officers, directors, employees, agents, volunteers, and representatives from any and all claims of injury caused by the negligence, if any, of the releases, or the negligence of a third party.
